A Case of Accidental Potassium induced Cardioplegia


Abstract
The artificial potassium cardioplegia has been studied by chest surgeons for the cardiac surgery since recent year. It is very serious that ultramicroscopic myocrdial injury, which was confirmed under normothermic potassium cardioplegia.
Recently, author has experienced the accidental potassium induced cardioplegia during anesthesia undergoing surgical managements for atonic bleeding.
Most of the causes of accidents were always careless managements of anesthesia. This accident has also brought by the injection of 10 o¢¥ KCI solution that volume of 10ml without Dr¢¥s confirmation.
In this case, the accidental potassium cardiac res.onses were so rapid and prompt with administration of KCI solution. It was also very serious and violent. And author had con-fused what to be done with it, because epinephrine use to bring hyper. potassemia which is the cause of this accident.
The patient had, however, resuscitated under closed cardiac massage and 100% OZ inhalation with administration of epinephrine hydrocortison CaC12 Sol. bicarbonate sol. and io% dextrose` solution which contains insulin.
The patient was rescued for 6 hours after resuscitation but unfortunately. expired. The cases of death are probably myocardial injury due to accidental hyperpotassemia and renal insufficiency which brought by the prolonged hypotension with massive bleeding before hospitalization.
